Why should I use the Health Assessment App?

The Health Assessment Application (App) enables you to securely select and complete a variety of self-assessments to help you manage your overall health.

How do I get my assessments to my VA care team?

Within the Health Assessment App, you can securely submit an assessment to your VA care team for review and to keep track of your progress.

Will the health care assessments within the App always be the same?

No. The assessments available will change over time and are planned to include various items that your care team may suggest you use.

How often should I take the assessments?

You should discuss the frequency that you should answer the assessments with your VA care team. You can receive reminders when you are due for a reassessment. You can set the App to remind you to take the reassessment on a weekly, biweekly, monthly or quarterly basis.

I started an assessment, but I do not have time to finish it. What should I do?

You can save the incomplete assessment by selecting the Save Draft button at the bottom of the screen.

How do I print a copy of my assessment?

Once you have answered all questions in the assessment, you can select the Print Responses option from the bottom of the screen.
